We are open! Opened February 2016. No, we do not accept any type of insurance; that's why we are a direct primary care practice.

IS THIS CONCIERGE MEDICINE?

No.  However, there is overlap between direct primary care and concierge medicine.  The most important difference is in the cost and billing structure.  

 

In general, concierge practices charge an annual retainer along with billing insurance.  The typical annual retainers are $1800 to $2400 per year.  

 

Direct primary care practices generally charge monthly fees which tend to be less than $100 and generally do not also bill insurance for other services, a value!

 

Both concierge medicine and direct primary care strive to improve the experience you have at the doctors office by making care more personal and taking more time to listen. 

Direct primary care is lovingly referred to as "working man's concierge."

DO YOU OFFER NON-MEMBER URGENT CARE VISITS?

We no longer offer these as the true intention of our clinic, direct primary care, is growing, and our time is devoted to our members. We did offer these in the past to try to help the community, but our focus has always about long term relationships with our patients. We do encourage you to consider our low cost medical clinic membership plans to see how they could help you with your potential urgent care needs.

DOES YOUR CLINIC FUNCTION AS AN "AS NEEDED" PRIMARY CARE CLINIC?

We do not function as an "as needed" clinic -- that would be the old traditional insurance payment model. We believe in a consistent relationship with our patients with our model of care, hence the low monthly cost to provide that.

IF I JOIN THE BARTLETT MEDICAL CLINIC & WELLNESS CENTER, DO I STILL NEED INSURANCE?

While we do not require it, we recommend you have it, yes. The Bartlett Medical Clinic & Wellness Center is not an insurance plan.  Your membership fees covers most primary care services but don't cover major surgery, specialists, or hospitalizations.  In general, direct primary care practices pair well with high deductible health plans or medical cost sharing plans. We recommend you consult an insurance broker to decide what plan makes the most sense for you. Further, we advise patients to consult in advance with an insurance broker and financial advisor regarding any use of HSA/FSA. You can learn more in detail here (DPC Frontier website).

 

You can typically use your HSA/FSA for billed labs, imaging or medication fees without issue.

HOW DOES DIRECT PRIMARY CARE WORK WITH RELATION TO TAXES/IRS?

Great question. We have a coalition of direct primary care doctors in America that are working together, at the state level and federal level, trying to help direct primary care grow by educating our legislators how true cost transparency starting at the level of the physician's office and then beyond, helps to control medical costs and burdens on not only our patients, but society. This coalition is working to change the IRS tax code to no longer be antiquated in the way the code was written BEFORE direct primary care even existed. It's called the Primary Care Enhancement Act. 

 

The Primary Care Enhancement Act (H.R. 365) clarifies to the tax code that DPC arrangements are not health plans for the purposes of the tax code, and defines fees paid to primary care providers in periodic fee arrangements as qualified health expenses. Please consider calling or writing your Representatives to ask them to actually learn about DPC, and this Act and to support it. This is how we work together to make actual care affordable for America.

HOW DOES YOUR BILLING WORK?

Immediately on the day (or next business day) of your enrollment online, you will pay a $99 enrollment fee per individual (family max $249). This is non-refundable after 5 days, or first office visit, whichever comes first. Two (2) forms of payment must be on file at all times (sorry, no pre-paid cards accepted).

 

Then 30 days later your billing starts for membership/medical services, thus paid in arrears. We bill on the 1st, 5th, 10th, 15th, 20th, 25th of every month. Just pick one of those days closest to 30 days, or we will adjust for you (unless you select a day prior to that 30 day mark that works better for your budgeting/pay periods).

 

MUST I BE SEEN IMMEDIATELY AFTER ENROLLING?

 

To be a member, no, you do not have to been seen immediately -- this reserves your place in our ~600 member total, but your billing starts effective immediately to be part of that ~600. 

 

For you to be a patient, you must be seen in the clinic at least 1 time before, for example, you could have medication refilled. We advise to not wait to get your first appointment, as those new patient appointments are not guaranteed same or next day.

WHAT IF I DECIDE TO CANCEL?  

We have no long term contracts or commitments.  If you decide to cancel, it's easy, you just let us know with 30 day written notice. Then, your last bill/payment will be at the end of that current billing period as we bill in arrears (meaning at the end of a 30 day period/cycle).

 

We do charge an enrollment fee and if you cancel and then decide to rejoin, you will be asked to pay the $149 per individual (family max $249) 

re-enrollment fee. Of course, if our panel becomes full in the interim, you may be wait listed to rejoin. Ability to re-join practice is at physician's discretion.

 

UPDATE 01/01/2018: If the above is not followed, or somebody disputes (accidental or intentional) our charges, they will then be liable for any dispute fees incurred by the Merchant Service Vendor (usually between $15 to $30 per dispute), Membership immediately terminated, and will then be sent to collections.

CAN I JOIN YOUR CLINIC, SEE YOU ONCE, AND THE GET 1 YEAR (OR MULTIPLE REFILLS) OF MEDICATION REFILLS?

No. This is simply not how we run our clinic, or our patient's care.

But I'm sure there are plenty of other doctors that would be happy to help you. Please consider alternatives avenues of care. 

WHAT IS INCLUDED FOR CHILDREN AGES 8 AND UP?

  • Care available starting at ages 8 and up, and must be vaccinated through at least age 6 vaccine series. See this page for help on vaccination.

  • Sick and urgent visits. (Well child visits not currently offered.) 

  • Markedly improved access with no copays. Access includes extended office visits, e-mail, text, phone, and video chat.

  • Any exclusions of care or other questions, review here

WHAT IS INCLUDED FOR ADULTS?

  • Markedly improved access with no copays. Access includes extended office visits, e-mail, text, phone, and video chat.

  • Adult annual physicals including completion of single page school and work forms (ex: sick excuse).

  • Sick and urgent visits.

  • Discounted labs & medications.

  • Preventive care including counseling on avoiding heart attack, stroke, cancer, and more.

  • Health coaching for weight loss, quitting smoking, diet improvement, lowering blood pressure, and more. (Advanced counseling requires Premium Plus Membership.)

  • Management of chronic diseases such as diabetes, and hypertension.

  • Coordination of care with specialists.

  • Depression and anxiety management.

  • In-office testing -- EKG, spirometry, pulse oximetry, urine dipstick, rapid strep, pregnancy test, basic hearing screen, vision screen.

  • In-office procedures -- suture removal, ear wax removal, abscess drainage, management of minor nose bleeds. (Add'l fees for Minimalist Primary Care.)

  • Single page form completion (ex: sick excuse).

  • Any exclusions of care or other questions, review here

HOW DOES DIRECT PRIMARY CARE WORK WITH REFERRALS? HMO/PPO?

As with anything insurance related, sometimes you don't know what will happen until you use it. That's true whether you use a traditional insurance based doctor, or our clinic (direct primary care). You need to take into consideration your insurance status as an HMO or PPO. Typically, direct primary care works better with PPOs for specialist referrals, however there is no guarantee that it will work.

We are always "out of network" providers, as we have elected to not participate in any insurance plans. Careful individual consideration by yourself can only guide you in whether or not direct primary care is for you.

 

Prior authorization requirements typically are not able to be fulfilled by "out of network providers" (which includes Dr. Bartlett) for Medicare, Medicaid, and private insurance companies, but she will work hard to see if we can find an alternative cost saving solution if possible, but she can make no guarantees. 

WHAT ELSE IS INCLUDED IN MY MEMBERSHIP FEE?

Depends on the membership level you choose. WHAT IS INCLUDED/EXCLUDED WITH MY CHILD'S MEMBERSHIP?

Your child’s membership includes simple school/sports physicals and sick care, similar to such as those visits that would require minor care at an urgent care, as appropriate. As of currently, “well child visits” are not offered, but this could change in the future. Rarely, physician may recommend vaccines which are not available at the clinic, for example, updated tetanus after a laceration.

WHAT IS NOT AVAILABLE WITH YOUR CARE? EXCLUSIONS OF CARE?

 

The doctor does not manage chronic pain and does not prescribe chronic pain medications/management (including Low or Regular Dose Naltrexone). She does not have or keep any pain medication at the clinic. Dr. Bartlett agrees that we do have an opioid epidemic here in Ohio that needs to be addressed at multiple levels. She is willing to make referrals to pain management physician groups in the area for them to directly help you if medically appropriate, but our clinic will not prescribe or refill them. We do not do "one-time refills."

 

She also does not routinely prescribe chronic controlled substances, including anxiety medications such as benzodiazepines (Xanax, etc), sleep meds (Ambien, etc), or other controlled substances (Lyrica, etc). This is because these medications are heavily regulated, need substantial monitoring, have a high rate of diversion and for most patients, long term, are detrimental to their overall health and well-being. And as such, if Dr. Bartlett does not agree to continue/start a medication, then we also do not do "one-time refills."

 

The only possible exclusion for controlled substance prescribing is of ADHD/ADD evaluation (on a case by case basis) with prior psychiatry evaluation notes for review by the physician. There is absolutely no guarantee to refill. Again, if Dr. Bartlett does not agree to continue a medication, then we also do not do "one-time refills." We are not a fee-for-service same day clinic. We work hard to meet our member’s needs as they arise but do not offer ‘same-day’ or ‘one’time’ visits for non-members for primary care. We believe in long-term primary care relationships. It’s what we do best.

At present time, we are not offering Gynecologic care (well women exams/PAP). This may change in the future. We do however make referrals and recommendations for this for our patients.

Dr. Bartlett is a trained MD, not a DO, thereby she does not do osteopathic manipulation. DO trained physicians have specific training with this, which is a large differentiating factor between the 2 types of training backgrounds of MDs and DOs.

We do not do pregnancy care, Workers’ Compensation Injuries and No-Fault Auto Injuries, DOT physicals, and we are not the admitting doctor when our patients need hospitalized.

We do not prescribe or refill Testosterone, nor do we prescribe hormones by the direction of a pharmacist. If a patient needs testing, we are happy to help with affordable labs so the patient may follow up with a specialist regarding their endocrine needs. We do not do "one-time refills".

We limit the types of forms we complete to only single page forms, for example, sick visit work excuses.

DOES YOUR PRACTICE BELIEVE IN VACCINATIONS? 

 

Dr. Bartlett does believe in vaccinating yourself and your children. However, she does not offer vaccinations in the office, nor does she offer well child visits. You will need a pediatrician for your children for that purpose. She will not complete any school/sport physicals unless your children are up to date on vaccinations. We offer sick visits to try to save parents money.

DO YOU DRAW LABS IN HOUSE OR SEND PATIENTS TO LAB?

We send our patients to a lab for bloodwork to be drawn, OR we have a mobile service meet you at our clinic on Tues/Wed/Thurs.

 

Dr. Bartlett is keenly aware of the inconvenience when the wrong tube is sent to the lab so it cannot be processed appropriately when drawn in a physicians office.

 

To remove possibilities like that, that is why she sends patients to the lab, plus patients can go when timing is flexible for them. Nobody should have to be starving while fasting for a 3 pm lab -- that's ridiculous.

DO I REALLY HAVE AN UNLIMITED NUMBER OF TEXTS, PHONE CALLS, VIDEO CHATS, AND EMAILS WITH MY DOCTOR JUST BY PAYING MY MEMBERSHIP FEE?

Yes!

 

Texting is for urgent needs only. Email for everything else. And of course, call 911 or go to ER if medical emergency.

WHAT IF I NEED CARE ON THE WEEKEND?

If you have an urgent after-hours concern that you believe cannot wait for routine business hours, we will try to be available by phone to assist you. While we will come in on the weekend for urgent needs, we find that that need is very rare. We are not available on the weekends for routine needs or matters that can wait until normal weekday business hours. Our doctor is dedicated to your care and to a healthy, cultivated physician-patient relationship.

SO, NO MATTER WHEN I NEED YOU -- YOU'RE AVAILABLE?

No… not exactly. Dr. B is a primary care physician. This means we can address most problems, most of the time. We are also a very low cost medical clinic. We are cultivating something sustainable, enjoyable and transformative. And it requires a partnership with our patients. In this age of instant responses and digital access, we are striving for sustainability. We ask our patients to utilize our services, in person or electronically, when they need us. We also ask that if matters can wait until waking, business hours that they do.

WHAT IF I'M HAVING A MEDICAL EMERGENCY?

As is customary with any true medical emergency, CALL 911. If you are not having an emergency but feel you need prompt medical care, please call us.

I DON'T SEE THE DOCTOR OFTEN, WHY SHOULD I JOIN?

We believe that having a trusting relationship with your primary doctor is important, even if you are healthy.  We offer health coaching to help you continue to improve your health.  Should you ever get sick, we will be there to guide you. Further, the problem with healthcare is that nobody thinks they need it, UNTIL THEY NEED IT. Better to have your bases covered, than have to scramble when you already aren't feeling well.

I HAVE MEDICARE, CAN I SIGN UP?

Absolutely!  We believe that our services are highly valuable to Medicare eligible people.  Our model allows us more time to listen to you and to coordinate your care with your other providers and caregivers. However, as a "Medicare opt-ed out" physician, Dr. Bartlett is required to have the following contract with you. Please review our contract with Medicare patients here

 

Dr. Bartlett and the Medicare beneficiary/patient are not allowed at anytime to try to seek reimbursement from Medicare for our any of our services. This contract (above) makes this clear. 

I HAVE MEDICAID, CAN I SIGN UP?

Absolutely!  We believe that our services are highly valuable to Medicaid eligible people.  Our model allows us more time to listen to you. However, Medicaid acts sort of like an HMO, if you're out of network as a physician, then you don't exist to them (see below)

Please note: Medicaid typically will not pay for (cover) prescriptions, services, or referrals that are written/referred by non-Medicaid providers, which includes Dr. Bartlett.

DO YOU ACCEPT PATIENTS WITH PRE-EXISTING CONDITIONS?

Yes.  We accept patients without any limits on pre-existing conditions. However, we do not prescribe chronic controlled substances with the exclusion of ADHD evaluation (on a case by case basis) with prior psychiatry evaluation notes for review by the physician. DO YOU OFFER TELEHEALTH SERVICES?

Yes!  Our EMR, AtlasMD, uses a platform that will allow us to securely text, video chat and more.  We will always strive to bring you appropriate care in the appropriate way.  If we can use our tele-health services to help save you a trip to the office after you have established care with at least 1 in office visit, we most definitely will.  

WHAT ABOUT SPECIALTY CARE?

The great thing about your primary doctor having more time, is that your primary doctor can solve more of your problems. In the direct primary care model, we can manage the majority of your healthcare needs.  

 

That said, clearly sometimes specialty care is needed, we have a large community of specialists that we work with. We will communicate carefully and consistently with specialists to be sure you receive an appropriate consult. 

DO/WILL YOU CREATE ITEMIZED BILLS FOR INSURANCE, CAR WRECKS, ETC?

No. That's not how direct primary care works. And no, we cannot just make some up, that's insurance fraud.
